Brain-sensing synNotch CAR T cells clear diffuse midline glioma in mice, but delivery route matters

  • A brevican-sensing synNotch receptor switches on a CAR against EphA2, IL-13Rα2 or B7-H3 only inside the brain, aiming to limit off-tumor toxicity in diffuse midline glioma.
  • In immunodeficient mouse models, intravenous or intracerebroventricular dosing gave tumor infiltration, persistence, antitumor activity and longer survival.
  • The B7-H3 version was fatally neurotoxic when given intracerebroventricularly, reflecting low normal-brain B7-H3 and fast CNS exposure; intravenous dosing was effective.
  • In an immunocompetent mouse model, a murine B7-H3 version cleared tumors durably and resisted rechallenge.
💡 Points Based on the abstract

The B7-H3 arm is the lesson: a gate on CAR expression doesn't help when the gate leaks in the wrong compartment. Fatal neurotoxicity after intracerebroventricular dosing, yet efficacy intravenously, says route and exposure kinetics set the safety margin, not just the logic circuit. Before any IND talk I'd want immunocompetent safety data with human-relevant antigen expression, and brevican-gate leakage measured outside the brain.
